Our goal is to provide the highest quality of care for your children and adolescents.  We want you to become familiar with some of our practice policies to assure your general understanding of how we run our office.  Of course, our friendly staff is always available to answer your questions.  To maximize your experience using our Electronic Medical Record please make sure to provide us with your email address.

  • Office hours are Monday thru Friday, 8 am – 5 pm for routine well child care and sick visits.  Emergency advice is available after 5 pm and weekends.
  • A parent/legal guardian must accompany child (children) for all appointments.  If a child is brought to our office for a scheduled appointment without a parent/legal guardian, the appointment will be cancelled and rescheduled according to normal scheduling protocol. Any patient under the age of 18 years of age must be accompanied by parent/legal guardian.
  • When calling our office and leaving a message, please be sure to leave your name, the full name of your child, his/her date of birth and only one (1) phone number where you can be reached.  We will return your call as quickly as possible.
  • Most of the time, you can schedule a sick appointment on the same day your child is ill.   When your child is ill, it is best to call early in the day.  A staff member will ask for the name of the child, the date of birth and the nature of the illness.  One of our pediatric RN’s will call you back to determine if you need to schedule an appointment to be seen by one of our doctors or nurse practitioners.  A sick appointment will be scheduled with whichever doctor or nurse practitioner who has an opening, please do not request one particular doctor unless it is an ongoing illness that one particular doctor is following.
  • Pediatric Associates schedules by appointment only.   We will not be able to see walk-in patients.  Questions for our pediatric nurses are by “call-in” only.  All calls will be returned in the order in which they are received.
  • We can schedule a maximum of 2 children per family, per well childcare appointment.  Likewise, if only one child is scheduled for an appointment, we cannot see a 2nd child at that time.
  • Yearly well check-ups, sports physicals and school physicals should be scheduled at least 2 to 3 months in advance.  The best time of year to schedule sports and school physicals is March – June.
  • We need 24 hour notice if you must cancel or reschedule an appointment.  You will be charged a $25 no-show fee if you do not show up for your appointment and it was not cancelled or rescheduled.  Chronic no-shows or broken appointments will be asked to find another practice to care for their children’s health and wellness.
  • As a courtesy, we will call 24 hours in advance as a reminder of a scheduled well visit.  All “well child” appointments must be confirmed by calling 856-691-8585 ext 102.  All “well child” visits must be accompanied by parent/legal guardian.  Unconfirmed appointments will be cancelled.
  • Please arrive 15 minutes prior to your scheduled appointment to complete/update insurance information and personal demographics.  It is most important to keep us informed of any changes in your contact information.  Additionally, all insurance cards must be presented at the time of the appointment. Pediatric Associates must be authenticated as the primary care physician (PCP) in order to be seen.
  • Due to HIPAA privacy regulations, all children, birth to 18 years, can be seen only when accompanied by an adult listed on their completed HIPAA paperwork.
  • All new/old patients must bring in current immunization records and pertinent health information to each scheduled appointment.  If we do not have records from previous doctor, we will not be able to schedule an appointment.
  • There is a form completion charge of $10 per form.  Copies of immunization records are provided at no charge through the portal.  There will be a charge of $1.00 per page for all patient requests for medical records (maximum $100).   Requests for medical records to be sent to another provider as a result of a transfer must be authorized in writing and will be released to the other provider at no charge.  Co-pays are required to be paid at time of visit.  An additional charge of $10 will be assessed if the co-pays are billed.
  • Newborn packets are given to all new babies at our practice.  The information in the packet is provided to keep you informed about your child’s growth and development and how to keep him/her safe.  It is important to bring this packet with you to all future appointments.
  • Most insurance is accepted, including Medicaid.  Please contact your insurance carrier to determine if we are participants.  Co-pays are mandated by your insurance carrier and we are required to collect them.  All co-payments are collected when services are rendered.  We accept Visa, MasterCard and Debit Cards in addition to cash, check or money orders.  There will be a $25 charge for all returned checks.  The person/parent bringing a child for an appointment is responsible for the co-pay that day.
  • Any questions regarding patient statements should be directed to our Billing Department Monday through Friday between the hours of 8 a.m. and 4:30 p.m.
